H1N1 Vaccine Information for Parents


The Hampshire Regional School District is working with local boards of health to provide parents the option of vaccinating their child against the H1N1 Flu at each school. Once the vaccine is received locally, schools will notify families of the date and time of each clinic.

Although the H1N1 vaccine can be administered either by injection (shot) or nasally as Flumist, we will not know what form(s) of the vaccine the schools will receive until the actual vaccine shipment arrives. If you have questions about either form of the vaccine for your child, it is important for you to consult with your child's physician before the clinic dates.

Below are links about the vaccine, as well as the Vaccination Consent Form, which will allow you to accept or refuse the vaccine for your child. There is no charge for the vaccine, but it can only be given to students who have written permission from their parent/guardian. Telephone permissions can not be accepted. Please review the Vaccination Consent Form and return it by Tuesday, November 3, 2009 .
