QUESTIONS? HERE ARE THE ANSWERS!

What information is needed in my child's health record?

When a student starts school at Hampshire Regional High School a health record is sent from their previous school. This includes immunizations, physicals, and any other health info that will help us provide for the student's health needs. Incoming seventh graders must have a recent copy of a physical with updated immunizations on file prior to the start of school. This is a Massachusetts Department of Public Health regulation for starting 7th grade. If your student is an athlete, a current physical must be on file prior to the start of that season's practice.

What if my student is on medication?

According the Massachusetts Department of Public Health regulations, the following must be on file in your student's health record before medication may be administered at the school:
1. Signed consent by the parent/guardian to administer the medication.
2. Signed medication order by your student's physician. This order must be renewed as needed AND at the beginning of each school year. 3. Medication must be delivered, to the school, in a pharmacy or manufacturer-labeled container by you or a responsible adult, whom you designate. No more than a thirty-day supply should be delivered to the school, and the student may not transport medication to school.
4. If your student requires an Epi pen or an inhaler, please contact the nurse's office for paperwork. Students may carry their own Epi pen or inhaler if ordered by the physician. An additional Epi pen or inhaler (provided by your) will be stored in the nurse's office.
5. If your student is a diabetic, please contact the nurse's office for paperwork and instructions for your student. All diabetic supplies may be stored in the nurse's office.
6. There are only 2 exceptions to not having a physician's order for medication: A.Your student may be given Tylenol, Ibuprofen, Benadryl, Tums, Calamine lotion, or 1% hydrocortisone cream with a parent/guardian consent because there is a standing order from the school physician. Please refer to the Student Emergency Form for a list of standing medication orders. Please note that medications listed on the standing orders are for occasional use only. B.Your student has been prescribed an antibiotic for ten days or less. A written note giving consent for administering the medication must be sent in and the medication needed during school hours must be delivered to the nurse in the original labeled pharmacy bottle.

What type of screenings does the nurse provide?

Hearing, vision, height, weight and BMI screenings are completed for all 7th and 10th graders. Scoliosis screening is completed for all 7th, 8th, and 9th graders. A parent or guardian is notified if a referral is needed. Any referral must be signed by a physician and forwarded back to the nurse's office. Confidential BMI results will be sent to parents/guardians via mail.

What happens if my child is injured during the school day?

First aid is provided for any injured student. A parent is notified for any follow up care. Over the counter medications may be administered to students with parent/guardian consent. Please remember to fill out the Emergency form in September to ensure the most up to date information on your student is on file.

Is there any other communication needed with the school nurse?

As always, communication is the key to helping us with your student's health needs. We hope that your student's years at Hampshire Regional are happy and healthy ones. Please call if there are any questions or concerns that you have.

What information is needed to help the school nurse care for my student?

Any new and existing medical information is helpful. All information is kept confidential under state law.
