Where is the nurses' office located? The health suite is located on the first floor across from the cafeteria. A triage room where first aid needs are treated is located immediately to the left as you enter the health suite. There are three rest areas for students that are ill.
What requirements are needed for enrollment to 7th grade or a new student to Hampshire Regional High School?

Updated immunization record that MUST include Meningococcal and Tdap: No student, as defined in 105 CMR 220.400, shall attend without a certificate of immunization documenting that the child has been successfully immunized in accordance with current Department of Public Health (DPH) recommended schedules against diphtheria, tetanus, pertussis (whooping cough), poliomyelitis, measles, mumps, rubella, Haemophilus influenzae type b (Hib), hepatitis B, varicella and other communicable diseases as specified by the Department.

A physical exam signed by doctor dated within 1 year of enrollment date.

Paperwork can be mailed to the nurse's office over the summer. All mail will be stored in a locked, secure location until the health office is open in late August. Paperwork may be faxed at any time to 413-527-1728. **For further information click Incoming Student Requirements **

Protected health information, such as physicals, should be mailed, faxed or hand-delivered to the nurse's office. The Attorney General has ruled communication via email in the public domain is not confidential, and is considered a matter of public record.
